Contact Forms and Email
The contact service uses PHP and PHPMailer to process general inquiries and send email. The email provider is subject to final configuration; no SMTP provider has been selected. Online requests remain disabled until private SMTP configuration is complete. Please call the office while online requests are unavailable.
Once enabled, the service emails the office and sends an acknowledgement to the address you provide. An inquiry or acknowledgement is not an appointment confirmation.
Do not submit medical histories, diagnoses, Social Security numbers, insurance identifiers, or sensitive health information through general website forms or unsecured email. Contact the office by phone to discuss an appropriate way to share patient information.
Cookies and Analytics
The website stores your optional-cookie choice in local browser storage. The PHP contact service uses an essential session cookie to protect form requests and help prevent abuse. Essential security functions are separate from optional analytics.
Google Analytics 4 and Google Tag Manager identifiers are configured, but external loading is disabled pending practice review. Accepting optional cookies does not currently activate these services. If authorized and activated later, they are intended to load only after consent; form contents are not included in the site's allowlisted analytics events.
Use Cookie preferences in the footer to accept or reject optional analytics or change your choice later.
